Your task

You are a medical student. Discuss and demonstrate the agreed genitourinary examination on a clinical skills model. Present your findings and discuss appropriate next steps with the responsible clinician.

Starting information

Use a clinical skills model in an appropriately supervised teaching setting. Your partner can take the patient’s communication role.

Suggested practice time: 8–10 minutes for a focused run, followed by feedback. Allow longer to practise the full sequence. This is not an official examination timing.

Preparation · Consent and priorities

Agree supervision and any communication or chaperone support before starting. Use an appropriate genital clinical skills model in a supervised teaching setting, with a partner taking the communication role and a chaperone role agreed. Do not perform genital examination on a peer. Provide a cover, gloves and a light for simulated transillumination.

Abdominal component · Model examination

Abdominal component

Explains the abdominal component, checks comfort and inspects including the flanks.

Starts palpation away from pain; assesses tenderness, guarding, masses and relevant organs gently.

Assesses relevant percussion findings, bowel sounds and femoral bruits.

Model examination

Renews permission before the testicular and penile component. Key step

Inspects scrotal colour and swelling; uses transillumination as part of assessing a swelling.

Palpates size, shape, consistency, masses and epididymis; assesses testicular lie and tenderness.

Does not use Prehn’s sign or a present cremasteric reflex to exclude torsion. Key step

Inspects penile erythema, vesicles, ulcers and urethral discharge.

Selects urine or STI sampling for the history and local protocol.

Ask the candidate

How would you present your findings and their significance? Describe pain/swelling, lie, tenderness, mass location and any penile lesions or discharge. Explain urgent referral or follow-up; investigations must not delay urgent treatment when torsion is suspected.

What would you do next, and why? Explain the important uncertainty, a relevant next assessment and when help is needed. State what you could not assess.
