Preparation · History and priorities

Use an appropriate genital examination model, couch, cover, gloves and light, with a trained facilitator where needed. Practise consent and chaperone discussion verbally. No genital examination is performed on a peer; use only findings actually represented by the model or agreed material.

Abdomen

Positions comfortably, inspects the abdomen and watches for discomfort.

Starts palpation away from pain and assesses relevant tenderness, guarding, masses and abdominal organs.

Selects percussion for organs or ascites and auscultation of bowel sounds or bruits as appropriate.

Genital model assessment

Renews permission before genital examination; inspects scrotal colour and swelling and considers transillumination.

Assesses testicular size, shape, consistency, masses, lie and tenderness, including the epididymis.

Does not use Prehn’s sign or a present cremasteric reflex to exclude torsion. Key step

Inspects the penis for erythema, vesicles, ulcers and discharge.

Selects STI samples and urine testing according to the history and local protocol.

Explains the working differential, required investigations and any urgent referral.

Finish

Thanks the patient.

Restores comfort and offers help with dressing where needed.
